Terms of Service Agreement

I, (print name) __________________ agree to the Terms of Service of Furry Tails Grooming Salon & Spa. I have read each section and agree to all terms therein. 
Additionally I agree that in the unfortunate event that a natural disaster or similar emergency the staff of Furry Tails Grooming Salon & Spa is entrusted to use best judgment in caring for my pet and will not be held liable for consequences related to such decisions. 

I also authorize Furry Tails Grooming Salon & Spa staff to assume guardianship over my pets in their care until such time as I can safely take possession of my pets. 

If my pet(s) are seniors and I will sign a senior pet waiver.
