"WORRY FREE" Maintenance Plan

The Plan Covers

-  Service Trip Fees
-  Labor Charges
-  Customer Optical Network Terminal (ONT)
-  Network Interface Device (NID)
-  All Company Installed Fiber/Wiring
-  Service Drop (From Natural Wear)
-  Water & Moisture Related Damages (1 Yearly)
-  Pet or Rodent Damages (1 Yearly)
-  Fire Damages (1 Yearly)
-  Lighting/ AC Power Surges (1 Yearly)
-  All Associated Labor For Services & Devices Listed Above

The Plan Does Not Cover

-  Vandalism/ Misuse
-  Service Drop Damages Without Legally Required JULIE Dig Request Number

Costs
-  The Cost Of The "WORRY FREE" Maintenance Plan is $10.00 Per. Month.  Customer Must Be Enrolled For A Minimum Of Twelve Months.


Option 2

"PAY AS YOU GO" Maintenance Plan

Pay $5.00 Per. Month ONT Rental
Pay Service & Repair Fees on a Time and Materials Basis

-  $45.00 Trip Charge (Cost To Roll A Service Truck/ Technician For Non-CMTC Network Related Issues
-  $65.00 Per. Hr. Minimum 1 Hr. (Labor For Authorized Customer Network Repairs)
